Chapter Bi-Annual Report

All CBM Chapters are required to complete the Chapter Bi-Annual Report (formerly known as Chapter Information Form) to provide updated information regarding chapter membership (status and changes), annual reports, planned youth activities and local chapter initiatives sponsored throughout the calendar year. Please forward the completed report to the CBM National Office via e-mail at: info@cbmnational.org or by U.S. Postal Delivery, ATTN: Chief Operating Officer
